In a memorandum dated August 28, 2009, former ODMH Director Sandra Stephenson announced that the Ohio Department of Mental Health would discontinue reporting requirements for instruments identified with the Ohio Outcomes System.

Adult consumer instruments used for measurement in the Department's legacy outcomes system are public domain and available free of charge.
Youth outcomes instruments also can be downloaded at this site, but licensing and use information for Ohio Scales for Youth are available through a separate site.
Although the Department no longer provides technical support for implementation and use of these adult and youth outcomes instruments, staffs in the Office of Research and Evaluation are willing to assist with questions about legacy resources. Contact ORE-ODMH@mh.ohio.gov

The outcomes surveys and procedures to be used by agencies are available here. Out-of-state or for-profit users of the Ohio Scales for Youth can access the licensing agreement here as well. The surveys for adults are in the public domain and free to use.
