Outcomes Scoring Guidelines 
This document contains the guidelines that should be followed when scoring the subscales on the outcomes instruments. Vendor techologies used to process outcomes data should compute subscale scores according to these guidelines.

Community Functioning Scale Scoring Tools
Because the computation of the community functioning scale score is somewhat complex, the Ohio Department of Mental Health (ODMH) Outcomes Team has developed two tools to assist agencies in computing the score. One is used with the ODMH Data Entry & Reports Template and one is for providers not using the template. Links to the tools and instructions for their use are posted below. Please contact the Outcomes Support Team if you have questions regarding these tools.

The information in this document is designed to provide a quick validity check of reverse scoring for those agencies NOT using the ODMH Data Entry & Reports Template. Use the scenarios provided to make sure your technology is accurately scoring the responses on the Adult Consumer A and Adult Consumer B forms.
