Data Entry and Reports Template
The Ohio Department of Mental Health (ODMH) has developed a data entry template that allows provider agencies to enter data from the outcomes instruments into a database at the local level. During the data entry process, data are validated and records are stored in a database structured to meet the data specifications defined by ODMH. Using the template, provider agencies can export records to the board and can produce individualized, client-based care management reports for use in treatment sessions. The newest version of the template can always be found on the Outcomes web site. The tool to import data from an earlier version of the template can be used to ensure that you are using the most recent version of the template. NOTE: The zipped template files are large and may take a few minutes to download.

The reports generator is to be used in conjunction with the ODMH Data Entry & Reports Template. It was designed with input from clinicians and administrators with the goal of saving provider resources. It produces different one-page consumer level reports and several aggregate reports, as well as provides a means of importing demographic data from other sources. The report generator produces reports for all instruments used in the ODMH Outcomes System. The instructions guide you step by step through the process of installing the reports generator and linking it to the template. The instructions also include information on how to generate the reports and important notes about the limitations of the tool. NOTE: The zipped reports generator file is large and may take a few minutes to download. A new version of the report generator was released in July 2008 and contains the social connectedness report.
