Outcomes Instruments - English Versions

The Ohio Mental Health Consumer Outcomes System instruments can be duplicated as required by individual provider organizations. The instruments for adults are free for use within Ohio as well as the Ohio Youth Problems, Functioning and Satisfaction Scales. Out-of-state parties must sign a licensing agreement and will be charged a minimal fee for use of these copyrighted scales. Questions regarding the Ohio Scales instruments and/or licensing agreement should be directed to the developer of the scales, Ben M. Ogles, Ph.D., ogles@ohio.edu.
