Performance Improvement

Certified community mental health agencies are required to develop and implement performance improvement activities as part of the certification and/or accreditation process. In addition, certified agencies are required by the Ohio Administrative Code to submit a Performance Improvement survey to the Ohio Department of Mental Health (ODMH). Currently, ODMH requires a semi-annual submission.

Use of Restraint and Seclusion in Community Mental Health Services
These reports provide information about statewide trends in the use of restraint in residential treatment and outpatient community mental health settings.

Current Performance Improvement Survey

PI Survey #13 is now available. This survey covers the time period between January 1, 2011 through June 30, 2011. Please complete and return the survey template by July 22, 2011.
