Tips for Mental Health Evaluators

The following documentation should be included to support your PASSR evaluation:

1. Nursing Home or Passport applications
   1. The 3697 or 1028 Consumer Report from Aging (if applicable)
   2. Admission and consultations from the hospital (if applicable)
   3. Doctors orders (e.g., physical, cccupational or respiratory therapies, medications)
   4. Discharge plan and/or continuity of care plans from the hospital
   5. The most recent health &pPhysical (H&P)
   6. Pertinent interdisciplinary notes (if it supports your recommendation)

2. Resident reviews
   1. Admission and consultations from the hospital and/or the nursing home
   2. Doctors orders
   3. Discharge plan and /or continuity of care plans from the hospital
   4. An RN-signed MDS (Minimum Data Set)

Helpful tips for completing the PASRR Review Process

• Identify where the consumer is living currently
• Allow the submitter to complete the 3622 PASRR Identification Screen
• Do not re-fax medical records that ODMH has already faxed on resident review cases
• Write legibly and largely enough so the report can be read; some evaluators type their reports
• Focus your report on the consumer's current symptoms and behaviors
• Give a detailed mental status
